SIR: I herewith forward report of the Department of Sanitation for the month of March, 1907.
The report shows in general an excellent condition of health on the Isthmus' The number of men constantly sick among the employees has been steadily dropping since August, 1906. In August we had 33.72 men out of every thousand sick every day in hospital. This has'steadily fallen, month by month, until March, 1907, when we have for this figure 19.40 men. In other words, we had among the employees in March very little over half the sickness which we had in August, 1906. This is slightly lower than our sick rate for March, 1906, when we had 19.65.
This is an excellent sick rate and about as small as we ca hope ever to get it.
Among our 4,600 American whites we had only 2 deaths from disease, which would give us a rate for the year of 5.1.7 per thousand. Among our 1,200 American women and children living in comm~isSion quarters we had no deaths and 'very little sickness of any kind
As has been the case for the last year, pneumonia was the disease which caused the principal mortality. During March it caused 53 deaths among our employees, and these 53 deaths were all among our negro employees.
As in most former reports, the negro employees show the highest mortality and the American whites the smallest. This mortality for March was: For negro employees, 48.50; white employees, 15.69; American white employees, 10.34.
During the month we had -no quarantinable disease. The last case of yellow fever occurred in May, 19906.

GENERAL SANITATION.
CITY OF PANAMA.
The general sanitary condition of the city is excellent and shows steady improvement under the various forces at work. While there are few mosquitoes to be found, the work of preventing their propagation is being carried on by establishing better drainage, filling wells and cisterns, and destroying tanks. During the month 171 places were reported as having larvie present, and 2 persons were fined by the authorities for having had larve present three times. One fine was i imposed for persistence in keeping an insaitary yard.
The carts engaged in the free delivery of water wre discontinued at the close of the month, hydrants having been established in the several sections where water was furnish ed .
Both the morbidity and mortality have been gratifyingly. low. No special disease has prevailed to a marked degree, and no acute contagious disease has been reported, except 1 case of rubeola and several cases of pneumonia. Of the deaths reported-62 in number, the lowest for any one month since our occupation-19 occurred in Santo Tomas Hospital, in which patients are treated from the surrounding territory as well as from the city. The greatest number of deaths caused by any one disease were caused by pneumonia, there being 10 due to this disease, while tuberculosis caused 6, and to beriberi 3 deaths were due. For these three diseases thorough' disinfection is being done.
The steady improvement in the erection of buildings continues. Nineteen plans for new houses were 'approved during the month. Street cleaning and garbage collection:

SANITATION-CANAL ZONE.
MOSQUITO WORK AND MALARIA.
In the towns along the Canal Zone but few stegomyia are to be found at the present time, and we do not often find the adult stegomyia in the employees' quarters. A year and a half has passed without the occurrence of a single case of yellow fever in the Zone. Our work at the present time is largely concentrated on the antimalarial campaign. We have a force of carpenters at work repairing and protecting the screening on the buildings occupied by whites, and also the laborers' barracks. This force, has finished the work between the districts of Tabernilla and Bas Obispo, and will complete' the repair work to be done by about the time the rainy season arrives.
A large amount of drain tile has been laid during the month, and those districts which have given most malaria lately are now being subdrained. It is an error to conclude that malaria disappears with the arrival of the dry season and that the anopheles stop breeding during that season. A very good- example of the efficiency of the antimalarial work has been shown in the Empire District during the month of March. A force of 400 to 600 laborers are in unscreened tents near the low ground known as the Savanas. About three-quarters of a mile away there is a camp up on a hill where about 20 men live. The latter camp is-soon to be abandoned, and so it is out of the question t spend money on antimalarial work in that vicinity. The vicinity of the camp near the low ground is 'used for building purposes, and so receives attention from the Department of Sanitation.
For the past five weeks the record of laborers contracting malaria, and sent to hospital4rom each of these two camps, is as follows:

From the above it is evident that in a small force of 20 men
under ordinary conditions 9 of them were sent to the hospital in five weeks, whereas a camp twenty times as large with attention from this Division only sent 14 to the hospital during the same
period of time.
The employees (both white and colored) at La Boca and Corozal
are remarkably free from malaria, and although two years ago the malarial sick rate at these two settlements was relatively high month after month it is now as low as at any similar location near the eacoast in warm countries. During the period of five weeks ending March 30, La Boca, with a population of 815 employees, sent only 12 cases of malaria to the hospital; and Corozal, with a population of 750 employees (mostly Italian laborers), sent only 3 cases of malaria to the hospital, and went for three weeks without
a case.
During the past several weeks the malarial sick rate (i. e., hos->
pital cases) among all employees living in the Canal Zone has been about 1 percent per week. It should be remembered that in 1904, when blood examinations for the malarial parasite were made at the Zone settlements, more than 80 per cent of persons examined showe d malarial infection. At the present time there is very little malaria among the American employees, and still less among their families. 'The Gorgona district has given the highest malarial record during the year. Most of the hospital cases there came from the camps at Juan Grande and Caballo Viejo. A large amount of clearing. near these two camps has been accomplished, and the camps are now being kept effectively screened. During the past two weeks a decided improvement in malarial conditions at Juan Grande has occurred. In several instances a marked advance in / the malarial sick rate has followed the arrival of new laborers at
certain camps. This matter was investigated and in some cases the malarial 'atients sent to the hospital had only been on the Isthmus from one to four days.' As malarial incubation takes a longer period than the time mentioned, there is n'o doubt.that many similar cases were not contracted on the Isthmus. In fact, a rise in malaria occurred at Corozal when the Italian laborers now stationed there first came. Investigation showed that some of these Italisns had had malaria before arrival, and a recurrence was to be
expected.,,
The Italian and Spanish laborers do not remain in their screened
quarters at night until they are ready to retire. SMALLPOX.
Although on several recent occasions the European laborers arriving on the Isthmus have sailed from European ports where smallpox is present, and cases have appeared during and immediately after the voyage, yet up to the present time there has been no development of smallpox in the Canal Zone.
GENERAL CONDITIONS.
The general health conditions of the various camps in the Zone are satisfactory and preparations are more complete than at any previous time to meet the coming rainy season, which starts during April.

QUARANTINE SERVICE.
There is little new to report for the month, as the conditions in the Peruvian ports with regard to plagueremain about the same. The number of cases has not shown any diminution and infection in the different centers must be considered as severe as during the preceding month. There has-been a slight increase in the number of cases of smallpox in Lima and one or two other places in Peru.
Plague is officially reported as present in three places in Chile: Santiago, Taltal, and Antofagasta. The latter two report a number of cases of this disease.
Concerning Guayaquil it may be stated that no appreciable change has taken place. The latest bills of health report eleven deaths a week from yellow fever, showing g that the town and surrounding country are generally infected.
During the month no vessel has arrived with quarantinable diseases on board.

REPORT OF THE BOARD OF HEALTH LABORATORY.
The Work consisted of the weekly bacteriological examinations of the water supplies of Colon, Gbrgona, Empire, Rio Grande, Las Cascades, and Bas Obispo, with special examinations of the sources of supply at Empire and Las Cascadas, and examinations of 'water from the Paraiso Spring, Camp. Elliott, and the Las Cascadas sterilizer. The sample of water from the Paraiso Spring was excellent in its relative freedom from bacteria. The spring bubbles up from the base of an uninhabited hill and is protected by masonry from surface contamination. The spring suggested to me that artesian wells properly located would furnish as good a supply for, municipal use in districts where the construction of large reservoirs would be impracticable.
Determinations of chlorine are being made in the various water supplies, with a view of ascertaining the local content and its variations in the dry and rainy season, and as a check on examinations for possible sewage contamination.
Sanitary analyses of the water supplies have been made from Rio Grande and Empire. In these analyses free and albuminoid ammonia, chlorine, solids, loss on ignition, nitrates, and nitrites are determined.
Other examinations were those of milk, evaporated milk, soap, sodium salicylate, urine, stomach contents, pus, sputum, blood, cerebro-spinal fluid, quinine sulphate. Three blood cultures were made. B. Typhosus obtained in one case; one was negative, and one contained -a Gram staining nonencapsulated diplococcus.

Agglutination reactions, autopsies, staining of sections from autopsies and operating room, and preparation of a polychrome methylenblue stain were completed.
A satisfactory method of permanently staining leprae bacilli in
tissues has been developed. Previously it was impossible to make
* permanent mounts with the methods and reagents mentioned in the
literature.
Determinations in progress may be mentioned: Analysis of wire screening, malted milk, sodium salicylate, banana food products,
opium quinine tonic, ammonia error in urea, investigation of terminal infections and irregular fevers.
