SIR: I herewith'forward report of the Sanitary Department for the month of April.
As you will see, in general, the health conditions are good. The death rate varies as usual, being much larger among the negroes than the whites. Among our 5,484 white Americans connected with the Commission we had only 4 deaths, which would give us a yearly rate of 8.7 per thousand. The death rate among the employees at large has decreased as compared with March. This was due principally to a decrease in the number of cases of pneumonia, having had in March 53 deaths from pneumonia, and in April 40 deaths. Of the 40 deaths 39 occurred among our negroes. We had 21 deaths from typhoid fever, 20 occurring among our negroes and 1 among the whites; 12 deaths from malaria, 11 occurring among our negroes and I among the whites. Our admission rate at the hospitals was only about three-fourths in April of what it was-in March. The constantly sick rate was only 21 per thousand. This probably is as small a sick rate as could be found in a similar body of men anywhere.

SANITATION-CANAL ZONE.
MIOSQUITO WORK, MALARIA, AND CONDITIONS PERTAINING THERETO.
At the present time the absence of adult mosquitoes in the Canal Zone is quite noticeable. Of course at certain points the adult culex is present, although no near-by breeding places exist, and it is evident that the range of flight of the culex on the Isthmus is quite a long distance. It is a known fact that t, e range of flight of the culex is quite large in North America, and the ability of the culex to travel long distances appears to hold true on the Isthmus also.
The unscreened houses, as well as the possible mosquito breeding places, are being inspected each week for adult mosquitoes and mosquito larve. This work is done \by special inspector, and acts

as a check on the work of the District Inspectors. During the past month very few adult anopheles have been found in the various camps. Stegomyia, also, are very hard to find at the present time. The subdrainage of the swampy lands near settlements is progressing and satisfactory results are being obtained. The carpenter force is now in the Las Cascadas District making minor repairs on the screened barracks and protecting the screening already in place. Similar work in the Gorgona District has been completed.
There has been very little rain. The water in streams and ditches has' become sluggish and warm. Alga-e have grown very rapidly, and it has required continuous attention at such places to prevent anopheles breeding therein. It has even become necessary to put a boat on the Bas Obispo River so that its banks could be kept free from algge nd vegetable growth, which protects anopheles larv.? and makes it impossible for fish to get at them. As an enormous amount of material is 'being removed from.- the Canal prism the shape of its cross section is continuously changing. Although the Canal bottom is relatively fiat, yet the seepage water that collects thereon is kept confined to ditches of limited section, where it can be watched and the occurrence of mosquito life prevented. As the excavation progresses the location of ditches has to be changed, as the axis of, a drainage ditch of one week often becomes the center line of a railroad track the following week, and the water must be kept in motion, or, 'when stagnant, covered with oil. The ditches and streams in the bed of the Canal that need constant attention extend from Pedro Miguel to Matachin, a 'distance of about 11 'miles.
The reports from the District Physicians are received weekly and compared with previous weekly reports. Where the number of malarial cases in any section of a district increases, the matter is looked into at once to determine the possibility of the existence of local sources of anopheles breeding near the camp that is affected. By this means, it is possible to locate and eradicate the breeding or resting places of anopheles and keep malaria down. Among Commission employees 'the number of malarial patients now going to the hospital each week is' less than 1 per cent of the entire working force, including Americans, Europeans, and West Inlian. laborers.
During the past nine weeks at La Boca, where 815 employees live and work, only 14 cases of malaria have been sent to the hospital.
At Corozal, with a population of about 600 employees, mostly European laborers and Americans, only 7 malarial hospital cases occurred during the past nine weeks.
During April there has been a steady decrease of malaria at Paraiso and at Gorgona. These two districts give us a larger percentage of malaria than the other districts.'
The swamp near the native village of Rio Grande has been filled in and malaria is decreasing in the laborers' camps in the vicinity.

Also the large swamp near the Culebra railroad station is being gradually filled in, and this will improve the health conditions at the American settlement on the hill.
During May and June the swamps between Empire and Culebra are to be filled in to protect the camps at Lirio, Cerro, and also the white quarters at Empire.
CLOSETS.
A large number of new pit closets have been constructed. All closets are being washed. and'disinfected daily by this Department. Also all old closets are being relocated and earth banked up around them ready for the rainy season. They are all being made fly proof.
GENERAL CONDITIONS.
The general health conditions are good. There is not much sickness among the American and European laborers. Precautions are being taken to prevent the occurrence of pneumonia, dysentery, and typhoid. All quarters from which cases of these diseases are taken are fumigated and disinfected.
The attached table is a summary of the routine work accomplished in the Canal Zone during the month.

QUARANTINE SERVICE.
During the month the health conditions of Guayaquil and the ports of Peru haVe remainectabout the same. No dimunition in the number of cases of yellow fever in Guayaquil has occurred, as the latest bills of health show 13 deaths in two weeks. The plague conditions of. the ports of Peru show little improvement. There has been a decrease in the number of cases in Pacasmayo and an increase in the port of Paita and the adjacent district. This latter place is suffering more severely from plague this year than any time since the introduction in 1904.
During the month, information having been acquired that the vessels were going to a new wharf recently completed in Corinto, Nicaraguoa, it became necessary to advise the steamship companies that such was not considered a safe procedure and, if it was necessary for theseships to moor at the wharf at that place, itwould be necessary to disinfect them and hold to complete six days after completion of such fumigation, suggesting at the same time that the vessels anchor in the bay as formerly, which was considered a safe anchorage and would obviate the fumigation and detention .of the ships here. This matter hasbeen adjusted by the adoption of the suggestion given.
During the month one case of smallpox has developed in a person, passenger from Cartagena, who was held in quarantine at
Colon, and it was necessary to fumigate and detain one vessel from Guayaquil on account of having on board a case that was suspected of being yellow fever.
